Senior Medical and Dental
All senior clinical staff responsible for patient care at Grampians Health are credentialed. We follow Victoria’s Credentialing and Scope of Practice for Senior Medical Practitioners policy.
Overview
All medical and dental credentialing applications are reviewed by our Medical and Dental Credentialing and Scope of Practice Committee for endorsement. Initial approval is for one year, with renewals every three years.
We use Equifax for credentialing. You will receive a request from Equifax to complete your online application. Visit My Clinical Profile for more details.

Credentialing
Credentialing is the formal process of assessing a healthcare professional's qualifications, training, experience, and clinical competence in relation to their role. All medical practitioners who work on-site or provide advice to our clinics must be credentialed before starting. This includes all consultants working for Grampians Health or working in a contract/locum capacity.
Grampians Health coordinates Regional Medical and Dental Credentialing with the following health services:
- Beaufort Skipton Health Service
- East Grampians Health Service
- Maryborough and District Health Service
If you are working across multiple sites, you will only need to complete one application. The requirements for each health service you are working for will be included in the one notification.
Residential aged care appointments
General practitioners attending residential aged care (our Aged Care Communities) will be verified prior to entering facilities, rather than being credentialed.
Antenatal shared care general practitioners
Community-based general practitioners can participate in our shared antenatal care programs for low-risk pregnancies. They will be credentialed by the Medical and Dental Credentialing and Scope of Practice Committee.
Credentialed general practitioners
General practitioners wishing to practice independently in other areas of Grampians Health (e.g. Emergency Department, Obstetrics and Gynaecology) must meet the specialist credentialing requirements of the relevant specialty. Those practising under supervision as junior medical staff do not require credentialing.
Each clinical discipline has a defined scope of core clinical practice as outlined in the Grampians Health policy. Senior medical staff may also apply for an advanced Scope of Practice, which requires specific credentialing.
Core Scope of Practice: Routine clinical services that are part of the practitioner's usual training and qualifications.
Advanced Scope of Practice: Specific clinical areas requiring additional training or expertise, such as specialised procedures or treatments.

Credentialing requirements for a new application
- Application (through Equifax)
- Resume (updated within the last 12 months)
- Medical Qualifications (Primary Degree and Fellowship)
- Current Medical Indemnity Certificate (Certified or email directly to CredentialSOP@gh.org.au)
- Current Police Check (within the last 12 months)
- International Police Check (if lived overseas for 12 months or longer during the past 10 years) Fit 2 Work or AHPRA International Criminal History Check
- Current Continuous Professional Development Certificate
- Grampians Health provider number – per site (via form or online). New specialists also need to complete the Specialist Recognition form
- Prescriber number
- Two references (preferred from within the speciality applied for and worked with for more than 3 months)
- Victorian Working with Children Check. Add Grampians Health to your Vic WWC and send/upload a copy with Grampians Health added.

Prior to commencement, senior clinical staff will need to return and complete all required documentation to People, Safety and Engagement; Infection Control; and Credentialing.
Continuous Medical Education
We offer Continuous Medical Education (CME) to our employed senior medical staff at all campuses. The approval amount is based on the Victoria Hospitals Industry Association CME support and implementation guide, and is then pro-rated against your FTE.
Your CME claims must be based on educational activities to maintain and enhance your knowledge, skills and competency to stay up to date with and understand the latest medical advancements and treatments.
