Our Adult Community Mental Health service provides care and treatment for adults in crisis or living with a severe and enduring mental condition. It is a specialist, recovery-oriented stream dedicated to supporting the mental health and wellbeing of adults aged 25–64 years.

Who we care for
The Adult Community Mental Health and Wellbeing stream is part of the Grampians Mental Health and Wellbeing Service (GMHWS) at Grampians Health.
We provide short, medium and long-term care and treatment for people who are experiencing a mental health condition and for those with severe and enduring mental health conditions.
This service is available to eligible people across the Grampians region.
What you can expect
The Adult Community Team will provide a complete mental health assessment to determine the right supports and provide you with information about what to expect from the service.
We will work with you to develop a care plan tailored to your individual needs and undertake regular reviews. We work in partnership with the consumer and their family, using a recovery framework.
Referrals and access
- Referrals are to be made through the Access and Triage Service.
- Referrals can be made by families, carers and supporters, health professionals, community agencies, or by self-referral.
