Aged Care Assessment - Grampians (ACAG)

Aged Care Assessment Grampians (ACAG) is a Commonwealth-funded service that assesses the care needs of older people.

Who we care for

We can provide clinical and non-clinical assessments for the following people to determine eligibility to access appropriate care services. This includes approvals for Australian Government subsidised Aged Care services: 

  • people aged over 65 years
  • First Nations people aged over 50  

Our assessments respect the diversity of all individuals. 

What you can expect

We meet with you and your caregivers to assess your physical, psychological, medical, restorative, cultural, and social needs. After each assessment, we recommend the level of support and services you need.  Assessments can be completed in hospital or at your home, depending on your circumstances. 

An ACAG assessment may be needed to access Commonwealth-funded: 

  • Residential Aged Care 
  • Residential Respite Care 
  • Home Care Packages 
  • Flexible Care 
  • Transition Care Program 
  • Short-Term Restorative Care 
  • CHSP (Commonwealth Home Support Program) Services
