By Five Paediatric Project
The By Five Paediatric Project is a cross-sector collaboration between Grampians Health and Uniting, which provides telehealth co-consults with children and families, a local health professional and a paediatric doctor.
Through these co-consults paediatric doctors provide guidance and management to families and health practitioners regarding child health and development.
Who we care for
The service is available to children aged 0-18 years living within the Wimmera Southern Malee who require co-management with a paediatric specialist.
What you can expect
The By Five Paediatric Project consists of two core elements:
- Provision of telehealth co-consults
- Local professional capacity building activites
The project aims to support children and families early in their development trajectory, through direct access to specialist care in co-consultations and increased local health practitioner confidence in addressing early developmental needs.
Referrals and access
Any currently registered health professional who is willing to attend and engage in co-consultations can refer to the By Five Paediatric Project.
Referral forms can be requested and returned to the service coordinator at byfivewsm@gh.org.au.
Referrers must be willing to be active co-consulters and continue to act as the primary care clinician to referred children and families. More details, including a co-consultor agreement, will be provided at the time of referral.
Once a referral has been accepted, the co-consultor will be instructed to book an appointment through the By Five booking portal.
