For Health Professionals

Referrals need clinical information to be accepted. The essential details required as per the statewide referral guide are:  

• A 12 month referral addressed to Dr Tim Elliott (for Gastroenterology and IBD) and Dr Jong Chin (for liver patients) 
• Include in your referral:     
  • reason for referral  
  • duration of symptoms and current symptoms  
  • relevant pathology and imaging reports  
  • past medical history  
  • current medications and allergies  

There is no out of pocket expense for patients.
