Grampians Health at Home

Grampians Health at Home provides care to people in their home that would otherwise need to be delivered in a hospital. 

Who we care for

Grampians Health at Home cares for people with serious, chronic illnesses, as well as people recovering from surgery, illness or injury. Your hospital care team or Emergency Department may refer you to the program so you can continue your care at home. 

We look after all age groups, including midwifery and paediatrics. 

Home-Based Cancer Care

Grampians Health at Home offers a Home-Based Cancer Care service, delivering quality care to patients with cancer in the comfort of their own homes. This is delivered in partnership with our Cancer Day Unit (CDU). 

What you can expect

Our highly qualified team of nursing, allied health and medical professionals provide care for people recovering from illness, injury or surgery, and for those with chronic conditions, in a home setting that is peaceful, familiar and more comfortable for them. 

Referrals and access

  • Referrals are required by your doctor. 
  • Grampians Health at Home staff can be requested by a major Melbourne hospital to visit you at home.

Home-Based Cancer Care 

The Home-Based Cancer Care is currently available within a 35km radius of Ballarat. Wimmera Cancer Care at Home is currently in development and will treat patients at home in the near future. Eligibility will be determined by your oncologist and haematologist.
