The Heart Failure Clinic aims to improve symptoms and quality of life for people with heart failure and reduce the need for hospitalisation.

Who we care for

We provide outpatient care for patients with heart failure across the region.

What you can expect

Before your appointment, our clinic pharmacist will contact you to discuss your current medications and symptoms. These details will be provided to the doctor you are seeing, along with a report for you consultation.

Patients have access to a general physician, cardiologist, heart failure nurse practitioner, and general medical registrar.

Referrals and access

  • A referral from your GP can be addressed to Heart Failure Clinic - Clinical Lead and faxed to 03 5320 4812; or
  • A referral from your Grampians Health Inpatient Care team
  • Patients may be seen at the Alan Wolff Medical Clinic in Horsham on the physicians fortnightly visits
  • All referrals are received by the Ballarat Specialist Outpatients Clinic
