For Health Professionals

Referrals to this program are accepted from the hospital. The PAC referral will occur when a person is discharged. To refer: 

• Ballarat: Referral via email is preferable. If the client requires nursing care, please call to discuss their needs before discharge. 
• Stawell:  Referrals via RIMS, emails or fax. External referrals via email or fax. 
• Horsham: Referrals currently via RIMS or Opal referral forms. External referrals via email, RIMS, or fax. 
• Edenhope: Referrals through email or RIMS only. External referral through email.
