Referral process
- Inpatients are referred by Grampians Health medical or allied health staff
- Outpatient referrals can be mailed or faxed by a GP or allied health professional to Grampians Health Central Intake [LINK TO PAGE]
- Self-referrals can be made by people who have attended our service in the past, by calling Central Intake [LINK TO PAGE]
Referral criteria
When referring to the Prosthetics and Orthotic department, please include the following information:
- Diagnosis
- Date of injury / amputation OR congenital
- Prosthetic / orthotic history (if known)
- Prognosis / treatment plan if injury is acute / sub-acute: e.g. length of wear-time, weight bearing status, ROM
- Relevant clinical information: e.g. ROM, strength
- Relevant co-morbidities
