Referral process

  • Inpatients are referred by Grampians Health medical or allied health staff
  • Outpatient referrals can be mailed or faxed by a GP or allied health professional to Grampians Health Central Intake
  • Self-referrals can be made by people who have attended our service in the past, by calling Central Intake 

Referral criteria

When referring to the Prosthetics and Orthotic department, please include the following information: 

  • Diagnosis
  • Date of injury / amputation OR congenital
  • Prosthetic / orthotic history (if known)
  • Prognosis / treatment plan if injury is acute / sub-acute: e.g. length of wear-time, weight bearing status, ROM
  • Relevant clinical information: e.g. ROM, strength
  • Relevant co-morbidities
