Who we care for
The Youth Community Mental Health and Wellbeing stream is part of the Grampians Mental Health and Wellbeing Service (GMHWS) at Grampians Health.
We provide specialist, youth-centred care and treatment dedicated to supporting the mental health and wellbeing of young people aged 15-25 years.
This service is available to eligible people across the Grampians region.
What you can expect
Our aim is to work in partnership with young people, their families, and support networks to reduce the impact of mental health conditions, build resilience and promote recovery. We provide:
- mental health assessments
- short term treatment and care
- interventions for young people who are in crisis
- family-based interventions
- school and community liaison
- peer support
- clinically informed case management
- ongoing support for young people who may require longer periods of treatment
Referrals and access
- Referrals are to be made through the Access and Triage Service.
- Referrals can be made by families, carers and supporters, health professionals, community agencies, or by self-referral
