Grampians Region Population Health Plan
The Grampians Region Population Health Plan (GRPHP) 2023-2029 begins with a vision for the people of our region: that we together achieve the best health and well-being outcomes possible, and that progress towards that goal is made each year.
In 2025 the Grampians Region Population Health Plan 2023-2029 was reviewed and updated. The Grampians Region Population Health Plan – Version 2 (GRPHP-V2) 2025–2027 was developed as a two-year subplan of the Grampians Region Population Health Plan 2023–2029 (GRPHP-V1) with the same strategic perspective that demonstrate the GPHU’s focus of work for the next two years.
