Edenhope’s Health & Wellbeing HUB is an important ‘one-stop-shop’ for the community, offering local access to important wellbeing services under the one roof.

We are a community hub supporting the residents of Edenhope and surrounds.  

What you can expect

The Health & Wellbeing HUB provides a diverse range of services spanning various fields and specialties.  

We have a ‘no-wrong-door’ policy – the HUB is an accessible and reliable health and wellbeing centre, located in the town’s centre for ultimate convenience.

Our allied health team provides expert care and health support for people through a range of services.   

We currently offer the following services through the Health & Wellbeing HUB: 

Our Cancer Resource Nurse provides support, information and links to community and specialist cancer services, for individuals and families affected by cancer.  

They can help with: 

  • Support at diagnosis, during and after active treatment to navigate your health and emotional needs. 
  • Managing your diagnosis and treatment, and arranging referrals if required. 
  • Providing information and contacts for cancer services – locally, regionally and state-wide as necessary. 
  • Communicating on your behalf with your doctors and other health care providers to ensure your health needs are met. 
  • Facilitating cancer education for patients, community groups and health professionals. 

Please note our Cancer Resource Nurse cannot administer treatment, interpret or advise on diagnostic results or treatment recommendations, or provide direct clinical service.  

Contact 

The Cancer Resource Nurse can be contacted directly by people with a cancer diagnosis, family members and care givers. Healthcare professionals can also refer clients. This position is located at the Health & Wellbeing HUB.

Our Home Support program delivers a range of services to support your choice to remain living at home safely and with confidence as you get older.

In collaboration with the Royal Fling Doctor Service (RFDS), face-to-face and telehealth appointments are available at the Health & Wellbeing HUB every six weeks for psychology support.

The Rural Outreach Program provides free mental health and wellbeing support to people living in rural and regional communities across the Wimmera Southern Mallee Shires. Our team can come to you at home, on the farm, or anywhere you feel safe and comfortable. We can also provide support via phone or Telehealth.

Our social work team provides guidance and assistance to patients, families, and carers as they adjust and reduce the impacts of issues and changes due to a health condition, illness, disability or getting older.

The Health & Wellbeing HUB is a virtual outpost for The Orange Door, which provides specialised care and support for people experiencing, or at risk of experiencing, family violence. If you are seeking safety and support as a result of family violence, the HUB stocks a range of The Orange Door emergency supplies which can be provided immediately.

The Transition Care Program provides short-term care to foster independence in older people after a hospital stay. We aim to reduce the number of older people staying too long in hospital or being prematurely admitted into residential aged care.

Exercise programs are offered as part of our community programs in Edenhope. Programs cater to all ages and fitness levels.

Our Social Support Group connects people with our community. We encourage people to be independent and to stay active and connected through social activities. The Social Support Group is run out of the Edenhope Day Centre, which is based at the Elsie Bennett Community Centre.
