Rehabilitation is a process that helps people regain their health and abilities after an injury, illness, or other difficulty.

Our team of clinical and allied health experts care for people who need rehabilitation, physical reconditioning or cognitive therapy across the Grampians region.
What you can expect
Rehabilitation helps you regain your abilities to live life to the fullest. We can support you in:
- moving and regaining strength
- finding new ways of doing things
- recovering after an acute medical episode
- recovering from a stroke
- recovering from orthopaedic surgery
We take a multidisciplinary approach to care, which includes rehabilitation specialists in medical, nursing and allied health areas. We work with you and your family or carer to achieve your short-term and long-term goals.
Rehabilitation can help reduce the length of hospital stays and prevent re-admission to hospital.
Referrals and access
- Referrals are made by the treating doctor. The patient will be referred into this service as part of their treatment in hospital, if necessary.
- External referrals can be completed by GPs or other health professionals using the referral form below and emailing it to saw@gh.org.au
