Referral process

For all external referrals to either our Inpatient Rehabilitation Unit (IRP) or our Geriatric Evaluation and Management Unit (GEM) please complete the referral forms and email to saw@gh.org.au 

To discuss a referral prior to completing the form, please contact the Subacute Liaison Nurse on 0403 589 933.
